Extraction vs Non-Extraction in the Era of Advanced Aligner Biomechanics — Clinical Decision-Making Across Complex Cases

Adj. Prof. Dato’
Dr. How Kim Chuan.
Mastering Biomechanics for Superior Aligner Control
Aligner biomechanics are the key to predictable tooth movement and successful case outcomes. In this session, Dr. Willy Dayan will break down the core principles of aligner biomechanics in multiple dimensions: vertical sagittal, root movements, rotations and more. Controlling these forces effectively reduces unwanted movements and improve efficiency in clear aligner therapy. Attendees will gain a step-by-step approach to evaluating and refining biomechanics to optimize patient results.
Key Takeaways
How to see the i-ortho plan as a representation of forces to move teeth.
Reducing unpredictable movements with accurate force application mechanics.
How to control difficult tooth movements using attachments and staging.
Strategic Customized Treatment Planning in Complex Aligner Cases
Even the best-planned aligner cases can experience inefficiencies. In this session, Dr. Dayan will walk through real-world complex clinical cases that could encounter tracking issues, unwanted side effects, or poor compliance. Attendees will learn proven techniques to manage complex treatments such as: skeletal dysplasia treated non-surgically, multiple missing teeth rehabilitation, Cr-Co shifts, severe open bites and deep bites, and more. Learn how to treat these cases with great efficiency and good clinical outcomes.
Key Takeaways
Never Present Something, they have not asked me for first
This session is a small sample of one of the multiple patient communication strategies that build trust, promote treatment acceptance and great compliance. Dr. Dayan has decades-long experience in orthodontic consultation that makes patients into partners during their orthodontic treatment. Clients are excited about results and very compliant. This leads to faster and better results that are good for the patient and good for the practice.
Key Takeaways
Pushing the Limits
Expand your possibilities with Angel Aligner treatment combined with auxiliaries
This lecture will explore how the integration of auxiliaries can significantly enhance the scope and effectiveness of Angel Aligner treatment. By reviewing clinical protocols and case examples, we will highlight how auxiliaries can be used to address complex movements, improve predictability, and expand treatment possibilities beyond conventional aligner therapy.
Key Takeaways
Understand when and how to incorporate auxiliaries into aligner treatment
Learn practical clinical protocols to increase efficiency and predictability
Review challenging cases demonstrating expanded treatment possibilities
Digital Orthodontics Meets Mixed Dentition: The Clear Aligner Advantage using Angel KiD
This session will address the controversial management of the transverse dimension in the mixed dentition using clear aligners. We’ll clarify the differences between orthodontic (dentoalveolar) and orthopedic (skeletal) expansion, showing how to capitalize on aligner therapy in the mixed dentition. I will examine the rationale— why, when, and how—of using Angel KiD clear aligners for transverse arch development, including clinical evidence, practical application, and outcome stability.”
Key Takeaways
Clear aligners engage our digital-natives with comfort-driven, device-friendly orthodontic management, supporting compliance and improved oral hygiene.
Paediatric Orthodontics using Angel Aligner KiD: Clinical Applications and Considerations
This session explores an early intervention strategy for skeletal Class III malocclusion with anterior crossbite, focusing on treatment initiated in the primary dentition. The approach begins with the use of a functional mouthpiece-type appliance during the deciduous stage to improve tongue posture and anterior overbite. Particular attention is paid to the etiological factors contributing to anterior crossbite in early childhood, including airway obstruction, tongue dysfunction, and occlusal interference.
After eruption of the permanent incisors, the case is re-evaluated and treated with Angel Aligner KiD to promote maxillary protraction, arch expansion, and distalization of the mandibular molars. This growth-sensitive protocol aims to address skeletal discrepancies early and reduce the need for future orthognathic surgery. A representative clinical case will be presented to illustrate this stepwise, growth-oriented approach.
Key Takeaways
Identify etiological factors contributing to anterior crossbite in the primary dentition, including functional and skeletal components.
Understand the timing and indications for initiating Class III treatment in the deciduous stage.
Learn how to integrate functional appliances and Angel Aligner KiD in a staged, growth-adapted protocol for skeletal Class III correction.
Extraction vs Non-Extraction in the Era of Advanced Aligner Biomechanics — Clinical Decision-Making Across Complex Cases
This lecture will explore:
- Precision biomechanics in expansion vs extraction decision-making
- Distalization, vertical control, and skeletal camouflage using aligners
- Managing surgical Class III cases non-surgically
- Controlling vertical dimension in open bite cases
- Treating asymmetry with growth modulation and biomechanical planning
- When technology expands possibilities — and when biology sets limits
Rather than engaging in a simplistic extraction vs non-extraction debate, this keynote proposes a case-specific decision algorithm rooted in biology, facial aesthetics, and long-term stability.
Integrating Angel Aligner and TADs for the Management of Complex Orthodontic Cases
This presentation discusses how Angel Aligner, when combined with Temporary Anchorage Devices (TADs), can effectively manage complex orthodontic cases.
A brief introduction to TADs will be provided, followed by 4–5 case presentations demonstrating their clinical application with aligners. This combined approach addresses the limitations of clear aligner biomechanics by using skeletal anchorage to accelerate treatment, improve efficiency, shorten duration, and reduce complications. At the same time, it maintains aesthetics, enhances comfort and oral hygiene, and achieves stable, predictable outcomes. Ultimately, the integration of Angel Aligner and TADs can help avoid extractions or orthognathic surgery while meeting patients’ expectations.
Combination Orthodontic Treatment using Angel Aligner
In contemporary orthodontics, a wide range of treatment modalities are available, including labial fixed appliances, lingual appliances, and clear aligner therapy. Since the advent of aligners, the practice of orthodontic treatment has undergone a substantial transformation, and in Japan, aligner therapy has increasingly become a familiar and widely adopted option.
At our clinic, treatment planning is carried out with an emphasis on maximizing both efficiency and quality of care, with careful consideration of the most appropriate modality for each clinical situation. Angel Aligner have become an indispensable adjunct in this context. They are utilized not only in the detailing stage following fixed appliance therapy but, more recently, also in the preparatory phase prior to commencing wire-based treatment.
The purpose of this presentation is to outline the clinical strategies by which Angel Aligner are integrated into our daily practice and to highlight their role in enhancing treatment outcomes within a comprehensive orthodontic approach.
Complex Extraction Cases in Clear Aligner Treatment Aided by MIA
This presentation explores the application of microimplants in clear aligner treatment for complex extraction cases. It highlights how microimplants enhances precision in tooth movement, addresses challenges like anchorage loss, vertical control, and improves treatment efficiency and outcomes.
Clinical insights are shared to demonstrate the practical value of this combined approach.
